Katie Thurston Has Documented Her Chin Liposuction, and Explained Exactly Why She Is Doing It
The reality star, who is living with stage four breast cancer, says this is the first surgery she has chosen, and she is posting it for transparency rather than persuasion.
Katie Thurston has been sharing photographs from a cosmetic procedure this week, and the framing she has given it is more interesting than the procedure.
"After so many required medical surgeries, I decided to do one for myself," she wrote on Wednesday, September 9, announcing chin liposuction. "I'm all about transparency. Never had Botox. Never had filler. But today I did, in fact, have chin lipo."
Thurston, 35, announced a breast cancer diagnosis in February 2025. Within months the cancer had spread to her liver, taking her to stage four. She has since worked through a range of treatments, including several medications and a double mastectomy.
The recovery, posted in real time
By Thursday she was showing the aftermath rather than the result.
"24 hours apart. Still swollen and healing," she wrote on Instagram Stories, alongside before-and-after images.
"Stitches under my chin," she continued. "I had a tiny drain where that dot is by my ear. The drain was removed next day. I'll do a more in-depth video as I continue to learn things and see the progress over the next few weeks to months."
Another post showed her on a couch with bandages wrapped around her head, next to her dog. It is not a flattering image and it is not meant to be.

What she says it is and is not
She has been unusually careful about the message, and the care is the point.
"Only sharing my journey for transparency and for those who are interested," she wrote. "Not trying to influence you to do this or make a statement about what is and isn't beautiful."
The reasoning she gives goes further than the standard disclaimer.
"We often compare ourselves to others, and I think it's important to be open and honest about things like this so that we are being realistic," she wrote. "And if people want to remain private about what they do to feel better about themselves, that's OK."
The position is a coherent one and rarer than it should be. She is not arguing that everybody should disclose. She is arguing that in a world where cosmetic procedures are common and largely undiscussed, somebody being specific makes the comparison fairer for everyone else.
The context behind one for myself
That phrase carries most of the weight of this story, and it only makes sense against the rest of her year.
Thurston has spent nearly two years in a sequence of interventions she did not choose, on a schedule set by her illness. In June she decided to postpone a liver biopsy and a new medication, explicitly for her mental health.
"I am confidently going to decline both a liver biopsy and a new medication for another four weeks," she said at the time. She was switching to a different treatment and wanted to see its effect on her labs before adding more variables. She needed, she said, a break from the pokes and the surgeries.
Set beside that, an elective procedure is not vanity in any ordinary sense. It is the first time in a long while that she has been the one deciding what happens to her body, and on what day.
She married the comedian Jeff Arcuri in March 2025, a month after revealing her diagnosis, and he has shared updates on her health since.
None of which requires anyone to have an opinion about chin liposuction. What she has actually published is a fairly precise account of what it is like to be treated for a long time, and what it is worth to choose one thing.
